A full time Care Manager position is available to support the Care Management Department.
The qualified candidate must be a Registered Nurse licensed in the state of Pennsylvania or have a Bachelor’s Degree in social work or in a health/social service related field with a minimum of 3-5 years experience in a health care facility and must have excellent communication and organizational skills with the ability to promote teamwork within and across departmental boundaries. Knowledge of utilization processes preferred. Candidate must be cognizant of regulations and policies related to payors.
This position will use care management skills to facilitate processes on a daily basis to coordinate patient care across a continuum of care in a timely and cost-effective manner; will educate providers and staff regarding the effective allocation of health system resources and will assure appropriate utilization of these resources to promote high quality patient care for all patients admitted to the hospital and evaluating appropriateness of admission and continued stay using approved criteria; will insure appropriate reimbursement through ongoing communication to insurance companies; will coordinate input from all health care professionals necessary to best meet the patients’ physical, emotional and spiritual needs; will assist patients to make sure an optimum outcome is achieved through assessment of patient/family needs; will assist with formulation of a discharge plan to meet those needs, coordinating services needed; will use best practices to educate patient/ family on chronic care; will support other care management focus areas as needed, etc.
